I removed a field from the document type and I can't seem to remove it from the pages. Do I need to remove the reference directly from the database [and how]? The only way I can see how this happen was importing a doc type from 4.0 to 4.5 may be buggy or perhaps I shouldn't have done it.
I did attempt to republish and it remained; perhaps I needed to republish the entire site. Unfortunately, the instance of 4.5 has been retired and I can't do further testing at this time.

Do you mean it's still on the published version? If so that's expected, until you publish a node it wont be removed from the published version of it.
Deleting a property doesn't republish any nodes.
