The customer wish to have three radiobuttons, where the page shows for example department names. But "behind" these radiobuttons Doc2Form has to send to the departments e-mail addresses.

You can edit the source code in app_code to send based upon the property value. There is nothing in Doc2Form at present to accommodate this.
You could potentially create a custom data type that would store the value with the email, but not display that in the form.
