When trying to create new content, I click on Settings, then Document Types, but in the right pane, all that is visible is the "Start Here" page. There is no "Create" button.
Please state the version of your Umbraco instance.
Other than that, I don't think this varies much between versions (though it's always nice to be specific, save confusion), so you should just be able to:
Right-click Document Types -> Create
Voila.
I don't think there's ever been a 'Create' button in the right-hand pane, at least not since 4.5.2 onwards.
