Running the latest version of Forms on Umbraco 7.4.3
I can't get conditions to work on checkboxes or radio buttons. I have found this post:

Is this a known issue. I can't even see the condition rule in the source code now - though it was there originally.
I am having this problem for iPad specifically, both Chrome and Safari.
