It looks like the tree items in the contour tree for the "Form" folder and the "Workflow" folders that are underneath the forms are missing a unique node id. They end up with their names merging together when you flip from one section of the CMS to another if you have a form node expanded to show the sub-folders.

To replicate:

• Have at least one form in Contour, expand the form node and click on either "workflows" or "entries"
• Flip to a different section, e.g. "content"
• Flip back to contour
• You'll see that both the form and the workflows nodes are now called "FormsWorkflows"
• You can make it worse by continually flipping between contour and another section, doubling up on the name each time, it's even worse if you have more than one form node expanded.......

I had a similar problem with a custom section after upgraded it to work with 4.5, I just had to assign a unique id to the node that was being used to populate the tree.
