Oh i see. You can create a Member Group under Members section, from there you specify which member will have access to a specific content. But for this you will need a login as well.
Under Content section i suppose you will have 2 different nodes with the 2 forms respectively. You just have give the appropriate Public Access > Role Based Protection > Member Groups Role (user has been assigned to)
