    Umbraco Formulate Form Checkbox list not working as expected

    Hi Team,

    I am facing the same problem as listed in this link below. The checkbox list is not rendering as expected. It is showing a label and textarea and no option to select.

    • A screenshot of the issue.
    • A screenshot of how your form/field/data values are configured in the back office.
    • The template you are using (i.e., the Angular one or the plain JavaScript one).
    • The Umbraco version you are using.
    • Any JavaScript errors in the browser console.
